Abstract
El acné vulgar es una patología muy frecuente con alta prevalencia del 70 a 87%, con rangos en mujeres de 61% y 83% para edades de 12 años y 16 años respectivamente; y para varones hasta el 40% y 95% para 12 y 16 años respectivamente. Se produce por la interacción de factores patogénicos como la colonización por C. acnes, queratinización anómala, influencia hormonal por andrógenos e inflamación. Las recaídas reportadas después del tratamiento con isotretinoína oral varían entre el 20% al 60 %. según los esquemas utilizados, el tiempo de seguimiento de 1 a 3 años y las características de la población estudiada. El objetivo de este estudio es de establecer la tasa de recaída durante el primeros años de seguimiento postratamiento con isotretinoina oral e identificar los factores pronóstico asociados con la recaída como: edad menor de 16 años, uso previos de retinoides tópicos, sexo masculino, grado severo de acne y dosis acumulada menos de 120mg/kg. Esto sería una contribución valiosa para el manejo personalizado del acné. Los resultados podrían servir de guia a los dermatólogos en la implementación de estrategias preventivas para minimizar las recaídas, optimizando los regímenes de tratamiento y seguimiento; finalmente el trabajo podría abrir la puerta a nuevas investigaciones sobre tratamientos complementarios o modificaciones en la terapia con isotretinoína para reducir las recaídas.
Abstract Acne vulgaris is a very common pathology with a high prevalence of 70 to 87%, with ranges in women of 61% and 83% for ages 12 years and 16 years respectively; and for boys up to 40% and 95% for 12 and 16 years respectively. It is produced by the interaction of pathogenic factors such as colonization by C. acnes, abnormal keratinization, hormonal influence by androgens and inflammation. Reported falls after treatment with oral isotretinoin range from 20% to 60%. depending on the schemes used, the follow-up time of 1 to 3 years and the characteristics of the population studied. The objective of this study is to establish the relapse rate during the first years of follow-up after treatment with oral isotretinoin and identify the prognostic factors associated with relapse such as: age under 16 years, previous use of topical retinoids, male sex, severe grade. of acne and cumulative dose less than 120 mg/kg. This would be a valuable contribution to personalized acne management. The results could serve as a guide for dermatologists in implementing preventive strategies to minimize relapses, optimizing treatment and follow-up regimens; Finally, the work could open the door to new research on complementary treatments or modifications in isotretinoin therapy to reduce relapses.
