Abstract
La apendicitis aguda es la inflamación del apéndice cecal, el cual se encuentra en el extremo del ciego. La apendicitis aguda es una de las patologías más conocidas en las consultas médicas de todo el mundo; por la alta frecuencia de aparición y evolución se debería tener herramientas diagnosticas útiles que nos ayude de manera previa y más viable llegar al diagnóstico. El hallazgo depende de la evaluación real, la anamnesis en la entrevista y del examen físico para la realización de la historia clínica, así como de la utilización de escalas demostrativas. La escala de Alvarado (EA) es la más mundialmente utilizada en las áreas de emergencia, ya que muestra la mejor ejecución de aprobación. La puntuación AIR pretendía superar los inconvenientes de las distintas escalas, integrando factores clínicos con el valor de la proteína C reactiva (PCR).
Problema: ¿El score de Respuesta Inflamatoria de Apendicitis (AIR) tiene mayor valor diagnóstico que el score de Alvarado modificado para apendicitis aguda en el Hospital Regional Docente de Trujillo en el periodo 2019 - 2022?
Objetivo general: Determinar si la puntuación de Respuesta Inflamatoria de Apendicitis (AIR) es más diagnóstica que la puntuación revisada de Apendicitis Aguda de Alvarado.
Basándose en los resultados, se crea un algoritmo de diagnóstico que puede ser empleado por los pacientes que sufren de dolor abdominal que podría ser causado por la apendicitis. Asimismo, permite diferenciar con mayor precisión a los pacientes que requieren una cirugía de apendicectomía de emergencia de los que pueden optar por un tratamiento clínico o expectante.
ABSTRACT Acute appendicitis is the inflammation of the cecal appendix, which is located at the end of the cecum. Acute appendicitis is one of the most known pathologies in doctors' offices around the world; due to the high frequency of occurrence and evolution, it should have useful diagnostic tools that help us in a previous and more viable way to reach the diagnosis. The finding depends on the real evaluation, the anamnesis in the interview and the physical examination for the clinical history, as well as the use of demonstrative scales. The Alvarado (AS) scale is the most worldwide used in emergency areas, as it shows the best approval performance. The AIR score was intended to overcome the drawbacks of the different scales by integrating clinical factors with the C-reactive protein (CRP) value. Problem: Does the Appendicitis Inflammatory Response (AIR) score have greater diagnostic value than the modified Alvarado score for acute appendicitis in the Hospital Regional Docente de Trujillo in the period 2019 - 2022? General objective: To determine whether the Appendicitis Inflammatory Response (AIR) score is more diagnostic than the revised Alvarado Acute Appendicitis score. Based on the results, a diagnostic algorithm is created that can be employed by patients suffering from abdominal pain that could be caused by appendicitis. It also allows to differentiate more accurately patients who require emergency appendectomy surgery from those who may opt for clinical or expectant management.
