Detección fenotípica de carbapenemasas en cultivos de Acinetobacter baumannii proporcionados por un hospital de nivel III de Trujillo - Perú

Abstract
Acinetobacter baumannii resistente a carbapenémicos (CRAB) representa un riesgo significativo para la salud pública a nivel mundial. En este estudio, nuestro objetivo fue detectar mediante pruebas fenotípicas la producción de serinocarbapenemasas y/o metalocarbapenemasas en cultivos de A. baumannii obtenidos del Hospital Belén de Trujillo de junio a agosto del 2023, Perú. Para ello se recolectaron 20 cultivos de A. baumannii aislados de muestras clínicas, con sensibilidad media y resistente a carbapenémicos. Para la detección se empleó el Método Interno de Inactivación del Carbapenémico (iCIM). Posteriormente, se determinó el tipo de carbapenemasas mediante la prueba de discos combinados con EDTA (Ácido etilendiaminotetraacético) y PBA (Ácido fenil borónico). Mediante la iCIM se identificaron 17 aislados como positivos para la producción de carbapenemasas de los cuales 15 fueron productores de metalobetalactamasas, 13 productores de serinocarbapenemasas; a s vez se hallaron 12 coproductores de ambos tipos de carbapenemasas. En conclusión, se detectó 17 cultivos productores de carbapenemasas, de los cuales el 88.2% fueron productores de metalobetalactamasas, 76.5% productores de serinocarbapenemasas y un 71% coproductor de ambos tipos de carbapenemasas.
Carbapenem-resistant Acinetobacter baumannii (CRAB) represents a significant risk to public health worldwide. In this study, our objective was to determine by phenotypic tests the production of serine-carbapenemases and/or metallocarbapenemases in cultures of A. baumannii obtained from a Level III hospital in the district of Trujillo from June to August 2023, Peru. For this purpose, twenty cultures of A. baumannii isolated from clinical samples, with medium sensitivity and resistant to carbapenemics, were collected. The Internal Carbapenemics Inactivation Method (iCIM) was used for detection. Subsequently, the type of carbapenemases was determined by the combined disc test with EDTA (Ethylenediaminetetraacetic Acid) and PBA (Phenylboronic Acid). By iCIM, seventeen isolates were identified as positive for carbapenemase production, of which fifteen were producers of metallobetalactamases, thirteen producers of serine carbapenemases; twelve co-producers of both types of carbapenemases were also found. In conclusion, seventeen carbapenemase-producing cultures were detected, of which 88.2% were producers of metallobetalactamases, 76.5% producers of serine carbapenemases and 71% co-producers of both types of carbapenemases.
