Características clínicas y pronóstico de pacientes con síndrome metabólico infectados por SARS-COV-2

Abstract
Las enfermedades metabólicas constituyen un problema de salud pública global desde hace varias décadas, su incremento progresivo ha traído consigo el desarrollo de comorbilidades que han aumentado la morbimortalidad en la población en general, más aún en un contexto pandémico por SARS-CoV-2 en donde el proceso inflamatorio agudo se agrega a un sinnúmero de eventos inflamatorios crónicos. Como profesional de la salud, resulta inminente en mi proceso formativo el tener un mejor y amplio conocimiento del tema para poder dar solución a este complejo binomio patológico prevalente de síndrome metabólico – infección por SARS-CoV-2, pues en el día a día de mi proceso formativo en Medicina Interna realizo el manejo clínico de pacientes con esta afección, enfrentando retos asistenciales que requieren una adecuada preparación.
Con el advenimiento de la pandemia de la COVID-19 el mundo ha entrado en un nuevo escenario, el cual evidenció que el hecho de que la especie humana se hallase en la cúspide de la cadena alimenticia no implicaba necesariamente dejar de ser vulnerable, más aún en donde el síndrome metabólico daba una base de riesgo basal preexistente. Los millones de muertes acaecidos y el desnudamiento de las realidades sanitarias precarias existentes, más aún en los países de bajos recursos, ha generado una gran preocupación colectiva, en una realidad en la cual la pobreza, la discriminación, la inequidad social y la falta de acceso a los servicios de salud resultó ser el común denominador que incrementó exponencialmente los efectos de la infección y sus fatales desenlaces. Ante este hecho, resulta necesario e importante dar una respuesta concreta a esta afección de salud, teniendo en cuenta la debida atención del sufrimiento y las expectativas del paciente y el impacto en el entorno de la familia y la sociedad.
Resulta prioritario dilucidar con mayor detenimiento esta nueva enfermedad, así como sus procesos fisiopatológicos, las características clínicas que devienen de los mismos y el pronóstico de los pacientes con síndrome metabólico infectados por SARS-CoV-2, pues ello conlleva a un mayor conocimiento del proceso mismo y de los efectos considerables dentro de los eventos clínicos y la atención pormenorizada en este grupo de pacientes, que bajo estas condiciones presentan una mayor vulnerabilidad, teniendo en cuenta que la intervención oportuna y un adecuado cuidado reducirán los potenciales riesgos, teniendo así un control más efectivo de la pandemia. Si bien es cierto que la inmunización contra el SARS-CoV-2 ha mejorado en sobremanera la expectativa de la salud, no deja de ser preocupante el elevado riesgo de formas graves en pacientes con múltiples comorbilidades, las cuales, englobadas en el concepto de SM, constituyen aún un riesgo latente, que debe ser abordado oportuna y adecuadamente.

With the advent of the COVID-19 pandemic, the world has entered a new scenario, which showed that the fact that the human species was at the top of the food chain did not necessarily mean that it stopped being vulnerable, even more so. where metabolic syndrome gave a pre-existing baseline risk. The millions of deaths that have occurred and the exposure of the existing precarious health realities, even more so in low-resource countries, have generated great collective concern, in a reality in which poverty, discrimination, social inequality and lack of Access to health services turned out to be the common denominator that exponentially increased the effects of the infection and its fatal outcomes. Given this fact, it is necessary and important to provide a concrete response to this health condition, taking into account due attention to the patient's suffering and expectations and the impact on the family and society environment. It is a priority to elucidate this new disease in greater detail, as well as its pathophysiological processes, the clinical characteristics that arise from them and the prognosis of patients with metabolic syndrome infected by SARS-CoV-2, as this leads to greater knowledge of the process. itself and the considerable effects within the clinical events and detailed care in this group of patients, who under these conditions present greater vulnerability, taking into account that timely intervention and adequate care will reduce the potential risks, thus having control most effective of the pandemic. Although it is true that immunization against SARS-CoV-2 has greatly improved health expectations, the high risk of severe forms in patients with multiple comorbidities, which, included in the concept of MS, is still worrying. , still constitute a latent risk, which must be addressed timely and adequately.
