Valor de la escala hematológica de sepsis en el diagnóstico de sepsis neonatal en el Hospital Regional Docente Trujillo

Abstract
La septicemia neonatal se conoce por signos y síntomas sistémicos debido a una infección bacteriana generalizada con hemocultivo positivo en las primeras cuatro semanas de vida; el diagnóstico clínico preciso de la septicemia neonatal es una tarea compleja; los recuentos hematológicos anormales por separado, reactivos de fase aguda e inflamatorios, niveles de citoquinas, especialmente al comienzo de la enfermedad no son sensibles ni específicos. Objetivo: Determinar si la escala hematológica de sepsis tiene valor en el diagnóstico de sepsis neonatal en el Hospital Regional Docente Trujillo. Metodología: se realizó un estudio analítico, observacional, transversal, retrospectivo de pruebas diagnósticas, incluyendo 139 neonatos que recibieron atención medica en el Hospital Regional Docente de Trujillo entre enero de 2019 y diciembre de 2023, se obtuvo información mediante ficha de recolección de datos, todos los neonatos de cualquier edad gestacional nacidos en el Hospital Regional Docente de Trujillo, hemograma completo y diagnosticados con sepsis neonatal por hemocultivo positivo y se excluyeron a neonatos con aplasia medular, neonatos con trombocitosis, neonatos con hemorragia intraventricular, Neonatos con enterocolitis necrotizante, neonatos con malformaciones congénitas.

Abstract Neonatal septicemia is known by systemic signs and symptoms due to a generalized bacterial infection with positive blood culture in the first four weeks of life; The accurate clinical diagnosis of neonatal sepsis is a complex task; Separately abnormal hematological counts, acute phase and inflammatory reactive, cytokine levels, especially at the beginning of the disease are not sensitive or specific. Objective: Determine if hematological escalation of sepsis has value in the diagnosis of neonatal sepsis at the Trujillo Regional Teaching Hospital. Methodology: an analytical, observational, cross-sectional, retrospective study of diagnostic tests was carried out, including 139 neonates who received medical care at the Regional Teaching Hospital of Trujillo between January 2019 and December 2023, information was obtained through a data collection sheet, all neonates of any gestational age born at the Regional Teaching Hospital of Trujillo, complete blood count and diagnosed with neonatal sepsis by positive blood culture and neonates with spinal cord aplasia, neonates with thrombocytosis, neonates with intraventricular hemorrhage, neonates with necrotizing enterocolitis, and neonates with congenital malformations were excluded.
