Factores de riesgo asociados a enfermedad pulmonar intersticial difusa en pacientes con artritis reumatoide en el Hospital Regional Docente de Trujillo, 2015 – 2019

| dc.description.abstract | Identificar los factores de riesgo asociados a enfermedad pulmonar intersticial difusa en pacientes con artritis reumatoide atendidos en consulta externa del Hospital Regional Docente Trujillo durante el periodo 2015-2019. Material y Método: Se realizó un estudio de casos y controles, a partir de las historias clínicas de los pacientes con artritis reumatoide y la presencia o ausencia de EPID. Se evaluó los factores de riesgo siguiendo un análisis bivariada mediante la prueba chi cuadrado y un análisis multivariado donde se estimó el odds ratio. Resultados: En el estudio se incluyeron 42 casos y 126 controles, donde se encontró una mayor prevalencia en mujeres respecto a varones en ambos grupos. Los pacientes que desarrollaron EPID presentaron una relación estadísticamente significativa respecto al DAS28 > 3.2 y a la edad de inicio de la artritis reumatoide ≥60 años con una significancia p<0.05. No hubo una relación significativa respecto al sexo masculino, tiempo de evolución de la artritis reumatoide, tabaquismo y factor reumatoide positivo entre los casos y controles (p>0.05). El análisis multivariado reporto respecto a la edad de inicio ≥ 60 años un OR de 7.84 (IC 95% 3.48-17.6) y respecto al DAS28 >3.2 un OR de 4.12 (IC 95% 1.48-11.46). Conclusión: Los resultados de nuestro estudio apoyan a los factores de riesgo, edad de inicio ≥60 años y el DAS28 >3.2 y su asociación con el desarrollo de EPID. | |
| dc.description.abstract | Abstract Identify the risk factors associated with diffuse interstitial lung disease in patients with rheumatoid arthritis treated in external consultation of the Hospital Regional Docente de Trujillo during the period 2015-2019. Material and Method: A case-control study was carried out, based on the medical records of patients with rheumatoid arthritis and the presence or absence of ILD. Risk factors were evaluated following a bivariate analysis using the chi-square test and a multivariate analysis where the odds ratio was estimated. Results: The study included 42 cases and 126 controls, where a higher prevalence was found in women compared to men in both groups. Patients who developed ILD presented a statistically significant relationship with DAS28 > 3.2 and age at onset of rheumatoid arthritis ≥60 years with a significance of p < 0.05. There was no significant relationship with male sex, duration of rheumatoid arthritis, smoking and positive rheumatoid factor between cases and controls (p>0.05). The multivariate analysis reported with respect to age of onset ≥ 60 years an OR of 7.84 (95% CI 3.48-17.6) and with respect to DAS28 >3.2 an OR of 4.12 (95% CI 1.48-11.46).Conclusion: The results of our study support the risk factors, age at onset ≥60 years and DAS28 >3.2, and their association with the development of ILD. | |
