Hiperglucemia como factor predictor de mal estado funcional en pacientes con accidente cerebrovascular isquémico

dc.description.abstract | Determinar si la hiperglucemia en pacientes con ACV isquémico agudo es un factor predictor de mal estado funcional a los 6 meses. Materiales y método: Se realizó un estudio observacional, analítico, de cohorte, prospectivo, que incluyó 153 pacientes con ACV isquémico agudo del Hospital Belén de Trujillo en el periodo de octubre 2021 a junio 2023. El mal estado funcional a los 6 meses después del ACV se midió mediante la Escala de Rankin modificada (ERm) y se clasificó como bueno (ERm: 0–2) o malo (ERm: 3–6). Para el análisis de los datos se utilizó un modelo de análisis multivariable de regresión logística, ajustado por variables intervinientes. Resultados: Se encontró una asociación significativa entre mal estado funcional e hiperglucemia (OR, 27.12; IC 95% 9.74 - 75.51; p=0.0001), sexo femenino (OR 2.26; IC 95% 1.13 - 4.51; p=0.0200), hipercolesterolemia (OR 2.66; IC 95% 1.02 - 6.95; p=0.0407) y Diabetes Mellitus (OR 5.03; IC 95% 1.96 - 12.8; p=0.0003). En el análisis de regresión logística, la hiperglucemia (OR ajustado, 24.176; IC 95% 8.25 - 70.84; p=0.0001), hipercolesterolemia (OR ajustado, 4.279; IC 95% 1.33 - 13.76; p=0.0147) y Diabetes Mellitus (OR ajustado, 3.801; IC 95% 1.20 - 12.00; p=0.0228) fueron predictores independientes de mal estado funcional, mientras que el sexo femenino (OR ajustado, 1.277; IC 95% 0.51 - 3.19; p=0.6005) no mostró asociación significativa. Conclusiones: La hiperglucemia es un factor predictor independiente estadísticamente significativo de mal estado funcional a los 6 meses en pacientes con diagnóstico de ACV isquémico. | |
dc.description.abstract | Abstract To determine whether hyperglycemia in patients with acute ischemic stroke (AIS) is a predictor of poor functional status at 6 months. Materials and Methods: An observational, analytical, prospective, cohort study was conducted, including 153 acute AIS patients from Belén Hospital in Trujillo between October 2021 and June 2023. Poor functional status at 6 months post-stroke was measured using the modified Rankin Scale (mRS) and classified as good (mRS: 0–2) or poor (mRS: 3–6). Multivariable logistic regression analysis adjusted for confounding variables was used for data analysis.
